Failure
to advise the Benefits/Human Resource Services of changes such as marital
status, birth of a child, or lost spousal coverage within 31 days of the
event, will result in you, your spouse and/or dependents becoming late
applicants with late applicant restrictions being imposed. If an employee cancels benefit coverage during a leave of absence,
he/she as well as dependents will be late applicants when the employee
returns to work. The insurance carriers, not the
School District, impose restrictions.
Certificated
Employees Restrictions
Late Applicant-Dental
- During
the first 12 months, coverage will only include basic services to a
yearly maximum of $50 per person.
- During
the second 12 months, coverage for basic and major services is
provided.
- Orthodontic
services are provided after 24 months.
Late
Applicant-Extended Health Care
- Employees
must complete and provide the insurance carrier with an application of
"Good Health" for herself/himself as well as dependents. Benefits/
Human Resource Services will forward the employee an application.
- If
ASEBP requests further medical information, the employee is
responsible for any cost associated with producing the appropriate
medical evidence.
- If
ASEBP approves the employee's and/or dependent's application, the
coverage will commence on the date given by ASEBP.
Late
Application-Vision & Hearing Aid Care (coverage for regular maximum)
- One
third of the benefit during the first claim cycle (2 years)
- Two
thirds of the benefit during the second claim cycle (2 years)

Classified
employees and dependents who are late applicants must wait until the first
of the month following the date Benefits/Human Resource Services receives
notification before being eligible to participate in group plan benefits.
